What it costs, and who pays
The four ways an hour of therapy gets paid for in this region, why a published price tells you almost nothing, and the questions that settle it.
Four payment models, not one market
An hour with a mental-health professional in this region is paid for in one of four ways, and which one applies matters more than any advertised figure. It can be public care with a capped patient fee, as in Estonian outpatient psychiatry, Swedish primary care under the high-cost ceiling, or a Norwegian psychologist with a public agreement. It can be free at the point of use, as with municipal low-threshold services, mental-health nurses, student and occupational health, and Lithuanian mental health centres. It can be a subsidised private session, as under the Danish referral scheme or the Finnish Kela decision, where a public body pays a fixed share and you pay the remainder. Or it can be fully private, where you pay the whole fee. The same professional can be any of these depending on the arrangement, which is why the question to ask is never only "how much".
Why this page prints no prices
Private fees are set by each practitioner, they differ between and within cities, they change every year, and a figure copied into a web page is wrong somewhere the day it is published — which is exactly how people end up with a number that turns out to be half or double what they are asked to pay. What is stable is the structure. So this page gives you the structure and sends you to the two places where the real number lives: the practitioner’s own published price list, which every serious private practice has, and the public bodies that set and publish the capped fees, the ceilings and the reimbursement rates for each country. Both are on your own country page.
The questions that actually settle the cost
Ask them before the first appointment and in this order. Is this public, subsidised or fully private? If subsidised, what does the public body pay per session and what is my share? Does this count toward my country’s annual fee ceiling — the decisive question in Sweden and Norway. How long is a session and is the price per session or per hour, which are not the same thing. What is the cancellation policy and its notice period, which is where unexpected charges actually come from. Is there a fee for the first assessment, and for reports or certificates? And how many sessions is this likely to be, since a per-session price is not the cost of the treatment.
What genuinely moves the price
Session length, first: a fifty-minute hour and a ninety-minute session are different products. The practitioner’s level — a trainee in a supervised teaching clinic, a qualified psychologist, a specialist psychotherapist, a psychiatrist — is the second. Location, with capitals higher than smaller towns everywhere in the region. Format: group sessions cost a fraction of individual ones per session, and remote sessions are sometimes but not always cheaper. And whether the practice holds a public agreement or contract, which in Norway is the difference between a capped fee and a private one and in Estonia between the five-euro visit fee and the full private rate.
Making an expensive course cheaper without making it worse
Several levers exist and none of them involves choosing an unqualified practitioner. Fee ceilings and high-cost protections mean a heavy year has an upper bound in several of these countries, and it is worth knowing where yours is. Group therapy is much cheaper per session and for some problems is the better treatment rather than the lesser one. University and institute teaching clinics offer supervised trainees at a fraction of the rate. Spacing sessions to fortnightly at the maintenance stage is a legitimate clinical option to raise with the therapist. Occupational health and student health are free to the people they cover. And the funded tiers on the free and low-cost page should be checked before, not after, the private search.

Perguntas frequentes
How much does therapy cost in this region?
It depends on which of the four payment models applies — capped public fee, free at the point of use, subsidised private session, or fully private — and private fees are set by each practitioner and change yearly. Ask the practice for its published price list and check your country page for the capped fees and ceilings.
Why will this page not give me a number?
Because a copied private fee is wrong somewhere the day it is published, and being told the wrong number is worse than being told where the right one lives. The practitioner’s own price list and the public bodies that publish the caps and reimbursement rates are the two reliable sources.
What is the most important cost question?
Whether the session counts toward your country’s annual fee ceiling. In Sweden and Norway that single fact separates a fee you will stop paying from one you will not, and it depends on whether the practice holds a public agreement.
How can I make therapy cheaper without making it worse?
Use the fee ceiling if your country has one, consider group therapy, look at university and institute teaching clinics with supervised trainees, discuss fortnightly spacing at the maintenance stage, and check occupational health, student health and the funded tiers before searching privately.
