A guarantee with dates in it
Sweden is the country here that has put the waiting problem into law with named intervals rather than an aspiration. The care guarantee sets out contact with primary care on the same day, a medical assessment in primary care within a small number of days, a first visit in specialist care within three months, and the start of treatment within three months of the decision. The important part is not the elegance of the scheme but what it entitles you to do: if your region cannot meet the interval, it is obliged to help you get the care elsewhere, and the guarantee is a thing you can name at the desk rather than a promise you have to argue for.
Two licensed professions, not one
Sweden licenses psychotherapists as well as psychologists. That single legal fact changes the whole business of choosing: "legitimerad psykoterapeut" is a protected licence with a defined training route behind it, verifiable by name in the national register of licensed health-care staff, and not something a practitioner can award themselves. In most of the region psychotherapist is either a protected title with weaker verification or an entirely open word. In Sweden it is a licence, and the register is the check.
What you actually pay
Patient fees are set by each region rather than nationally, but they sit under a national high-cost ceiling: once your fees over a rolling twelve months reach the ceiling, the rest of that period is free. Most regions also make health care free for children and young people up to a defined age. The system is deliberately designed so that a year in which you need a lot of care does not become a year in which you cannot afford it — but the ceiling applies to public care, not to a private psychologist working outside any regional agreement, which is the distinction that catches people out.
Primary care is meant to hold this
A Swedish primary care centre is not only a doctor. Counsellors and psychologists are employed inside the vårdcentral, and much of the country’s mental-health work — assessment, brief therapy, sick-leave decisions, follow-up — is designed to happen there rather than in specialist psychiatry. You also choose your centre, and can change it. If the first centre cannot offer psychological treatment within a reasonable time, that is a specific question to ask another one, and it is a more productive question than asking for a referral into specialist care that a mild or moderate problem does not meet the threshold for.
Children have their own front line
For young people there is a deliberate two-step structure: a first-line service for mild and moderate problems, and child and adolescent psychiatry for the rest. Sending a child straight at specialist psychiatry when the first-line service is the right level tends to produce a long wait and a referral back. Several regions now allow self-referral into both, which is a genuine change from the older model where a school or a doctor had to open the door. The children and teenagers page covers what the school is separately obliged to provide.
If it goes wrong
Each region has a patients’ committee that helps with complaints and mediates, and the national inspectorate supervises health care and takes reports about care that has gone seriously wrong or about a practitioner’s fitness. Those are two different addresses for two different problems: the committee for "this went badly and I want it looked at", the inspectorate for "this person or this clinic should not be practising like this".

Preguntas frecuentes
What does the Swedish care guarantee actually promise?
Same-day contact with primary care, a medical assessment in primary care within a few days, a first visit in specialist care within three months and treatment within three months of the decision. If your region cannot meet the interval it must help you obtain the care elsewhere.
Is a Swedish psychotherapist licensed?
Yes. Sweden licenses both psychologists and psychotherapists, and both appear by name in the national register of licensed health-care staff kept by the National Board of Health and Welfare. That makes Sweden the easiest country in the region in which to verify a therapist.
How much does therapy cost in Sweden?
Inside public care you pay a regional patient fee that counts toward a national high-cost ceiling, after which care is free for the remainder of the rolling twelve months; many regions make care free for young people entirely. A private psychologist outside any regional agreement charges a full private fee that the ceiling does not touch.
Can I choose or change my primary care centre?
Yes. Choice of primary care centre is part of the system, and changing is a practical option if the centre you are with cannot offer psychological treatment within a reasonable time.
