What a psychotherapist is

A long post-qualification training in a named method, sat on top of another profession — and the one title in this field that is sometimes anybody’s to use.

A second training, not a first one

Psychotherapist is not an undergraduate destination. It is a further training of typically three to five years, part-time, undertaken by someone who already has a relevant base profession — most often a psychologist, a doctor, a psychiatric nurse or a social worker. The training has three components that make it long: theory in a specific method, several hundred hours of supervised clinical work with real patients, and, in most traditions, the trainee’s own therapy. That last requirement is unusual among health professions and it exists because the instrument being trained is the therapist.

The method is part of the qualification

Unlike psychologist or doctor, the title carries a school with it: cognitive-behavioural, psychodynamic, family and systemic, integrative, and so on. A qualified psychotherapist is qualified in something, not in psychotherapy in general, and the training bodies are usually modality-specific institutes rather than universities. When someone says they are a psychotherapist, the useful follow-up is which training, which institute and how long — not to catch them out, but because the answer tells you what the sessions will actually be like. The therapy types page covers what the main methods do differently.

Where the title is protected and where it is not

Finland grants psychotherapist as a protected professional title, verifiable in the public lookup. Sweden goes further and licenses it as a health-care profession, so a Swedish licensed psychotherapist appears in the national register alongside doctors and psychologists. Elsewhere in the region the protection is weaker or absent, and the word can be used by people with a weekend course behind them and by people with a decade of supervised practice, with nothing in the title to tell them apart. Where the state does not do the filtering, the professional bodies partly do: national umbrella associations and the European accreditation route accredit individual therapists and their training institutes, and membership of a recognised association is a real, checkable signal in exactly the countries where the title itself is not.

What to ask when the title proves nothing

Four questions settle it, and none of them is rude. Which base profession do you hold, and where is it registered? Which psychotherapy training did you complete, over what period, and with which institute? Do you have regular clinical supervision now? And what is your professional association or accreditation? A qualified therapist answers all four in a minute and is not offended by being asked; hesitation on the supervision question in particular is informative, because ongoing supervision is a professional standard rather than a sign of inexperience.

Counsellor, coach, therapist

These words are not interchangeable and only one of them is sometimes regulated. Counselling is usually shorter, more focused on a present difficulty and often excellent value for exactly that. Coaching is not health care at all, has no clinical training requirement anywhere in this region, and is not an appropriate response to a mental illness however it is marketed. A great deal of harm in this field comes from someone with a genuine depression spending six months and a lot of money on something that was never designed to treat one. The counselling and support page sets out where each fits.

Why any of this matters clinically

Research on what makes therapy work keeps returning the same answer: the relationship, the therapist’s competence and the fit between the problem and the method account for most of the difference, more than the brand of the method itself. That is an argument for caring about training and supervision rather than for shrugging at credentials. A well-trained, supervised therapist in a method that suits the problem is the thing worth searching for — and it is the only part of the search that is checkable in advance.

Preguntas frecuentes

Is psychotherapist a protected title?

In Finland it is a protected professional title and in Sweden it is a licensed health-care profession, so both are verifiable in a public register. In several other countries in the region the word is much less protected, which is why the training, the base profession and the association matter more there.

What should I ask a psychotherapist before booking?

Which base profession they hold and where it is registered, which psychotherapy training they completed and with which institute, whether they are in regular clinical supervision now, and which professional association or accreditation they hold. All four are ordinary questions.

Is a psychotherapist better than a psychologist?

They are different qualifications, not a ranking, and many people hold both. For long or complex work, specific psychotherapy training matters. For assessment, or for structured brief treatment of mild to moderate problems, a psychologist without separate psychotherapy training is entirely appropriate.

Is coaching the same as therapy?

No. Coaching is not health care, requires no clinical training in this region, and is not a treatment for a mental illness. It can be useful for goals and performance; it is the wrong tool for depression, anxiety disorders, trauma or anything that needs a diagnosis.

Who does what

What a psychologist does

Five or six years of university training, assessment as the core skill, no prescription pad — and a title that is protected in some of these countries and not in others.

What a psychiatrist does

A medical doctor with a specialist qualification: diagnosis, medication, physical causes, sick leave and hospital care — usually in shorter appointments than people expect.

The mental-health nurse

The fastest funded appointment in several of these countries, and the one most people have never heard of.

Counselling, peer support and everything that is not therapy

Helplines, counsellors, peer groups, chaplains, self-help programmes and employer schemes: what each is genuinely good at, and where each stops.
