Online and remote therapy

What remote sessions change, which country’s rules apply when the therapist is over a border, and where video is the wrong format.

The evidence is better than the reputation

Remote delivery of structured psychological treatment, particularly cognitive-behavioural work for anxiety and depression, performs comparably to the same treatment in a room in a substantial body of research, and guided internet-delivered programmes have their own evidence base. Finland has built public infrastructure on that finding. So the question is not whether remote therapy works but which problems it suits, and the answer to that is more specific than the marketing suggests.

Where remote is genuinely worse

A screen removes information that some treatments depend on. Assessment of a first, unclear presentation is harder without the room. Anything involving physical risk — active suicidality, self-harm, an eating disorder needing physical monitoring — needs a clinician who can see and act locally. Psychosis and severe confusion are poorly suited. Children, especially younger ones, are much harder to work with remotely. And exposure work in trauma or severe anxiety requires a therapist who can manage what happens after the camera goes off, which is exactly the moment a remote session ends. A good remote service will say this itself and will screen you out; one that accepts everybody is telling you something.

The border question

A therapist licensed in one country and working into another is a real arrangement with real limits. Public funding almost always follows the country’s own system — a Kela decision is for Kela-approved therapists, a Norwegian agreement practice is a Norwegian arrangement, a Danish referral buys a Danish subsidy — so treatment across a border is usually private treatment. Regulation follows the therapist’s registration, so if something goes wrong the complaint route is in their country, not yours. And practical matters follow geography: a therapist who cannot contact your local emergency services, does not know your health system and cannot write to your doctor is at a real disadvantage on the day it matters. None of this rules it out; all of it belongs in the decision.

Platforms and what they do not tell you

Commercial platforms vary enormously and the important facts are usually not on the landing page. Which country is your therapist registered in, and under which title? Is the same person assigned to you each week, or the next available one? Is the service therapy, coaching or messaging support, and does it say so anywhere in its terms? Where is your data stored, who processes it, and can the notes be exported to a clinician? Is there a crisis policy, and what does it actually do at three in the morning? A service that answers all five clearly is a different proposition from one that answers none.

Setting up a session that works

Small practical things carry more weight remotely than in a room. A door that closes, headphones, and a plan for who else is in the flat. A wired connection or a strong signal, because a dropped call in the middle of difficult material is its own event. Agreement in advance on what happens if the connection fails — who calls back, on what number. A phone number the therapist can actually reach you on, and the name of a local contact or service for emergencies. And your own end-of-session ritual: an in-person appointment ends with a walk home, and a video call ends with you sitting in the same chair, which is why people find remote sessions harder to put down.

Частые вопросы

Does online therapy work as well as in person?

For structured treatment of anxiety and depression, research finds comparable results, and guided internet-delivered programmes have their own evidence base. It is less suitable for unclear first assessments, active risk, eating disorders needing physical monitoring, psychosis and young children.

Can I see a therapist in another country?

Privately, usually yes. Public funding almost always follows your own country’s system, so cross-border treatment is normally paid for privately, and any complaint goes to the regulator where the therapist is registered rather than where you live.

What should I ask an online platform?

Which country your therapist is registered in and under which title, whether you get the same person each week, whether the service is therapy or coaching, where your data is stored and whether notes can be exported, and what the crisis policy actually does.

Is video better than telephone?

Not always. Video carries facial expression, which matters for some work; telephone is easier for people who find being observed difficult and is more robust on a poor connection. What matters more than the medium is a private space, an uninterrupted hour and a plan for a dropped call.
