The mental-health nurse

The fastest funded appointment in several of these countries, and the one most people have never heard of.

Why this page exists

Almost everyone searching for help in this region asks for a psychologist or a psychiatrist, because those are the two words the culture supplies. In Estonia, Finland, Lithuania and Norway there is a third profession sitting closer to the door, with a shorter queue and no fee: a nurse who has specialised in mental health and works inside primary care. In Estonia it is the mental-health nurse in the family health centre; in Finland the psychiatric nurse at the health centre or in occupational health; in Lithuania a member of the mental health centre team; in Norway part of the municipal mental-health service. Different names, same idea, and it is usually available in weeks rather than months.

What the role can actually do

More than the word nurse suggests. Structured assessment with validated instruments; psychoeducation, which is a plain term for explaining precisely what is happening to you and why the symptoms take the shape they do; structured brief interventions built on cognitive-behavioural methods; sleep and activity work; monitoring how a medication is settling and reporting back to the prescriber; and the practical scaffolding — sick leave paperwork, employer conversations, benefit forms, coordinating the other appointments — that keeps a life standing up while the treatment works. The last of those is undervalued and often decides whether the treatment gets a chance.

What it cannot do

It is not a prescriber and it is not a replacement for psychotherapy. If a diagnosis is unclear, if there is significant risk, if psychosis or bipolar disorder or an eating disorder is in the picture, or if medication needs to start or change, the nurse escalates rather than manages — and one of the strengths of the role is that this escalation happens inside the same service, without a new referral from outside. For a long modality-specific therapy, the nurse is the person who helps you get onto the right list, not the person who delivers it.

Who it fits best

It fits mild to moderate anxiety and depression, stress and burnout, sleep that has come apart, adjustment after a life event, and the very large group of people who are functioning but not well and do not know where to take that. It also fits the waiting period: if you have been referred for something slower, this is a real appointment to have in the meantime rather than an empty gesture, and it produces notes that make the later assessment better.

How to ask for it

The wording matters, because the receptionist is matching your words to an appointment type. Ask directly for the mental-health nurse, or the psychiatric nurse, at your health centre — not "an appointment about stress", which will be routed to a GP with a ten-minute slot. If the practice does not have one, ask what its equivalent is: in Norway ask about the municipal mental-health service and prompt mental health care, in Lithuania about the mental health centre for your area, in Sweden about the counsellor or psychologist at the primary care centre. The country pages linked from the hub say which name applies where.

Questions fréquentes

Is a mental-health nurse a real alternative to a psychologist?

For mild to moderate anxiety, depression, stress and sleep problems, it is a genuine treatment option rather than a holding measure — structured, evidence-based and usually free. For complex problems or long therapy, it is the route in rather than the treatment itself.

How do I book one?

Ask your health centre or primary care practice for the mental-health or psychiatric nurse by name, rather than asking for an appointment about stress. In Norway ask the municipality about its mental-health service; in Lithuania the mental health centre for your area is the equivalent.

Does it cost anything?

In Estonia it is covered by insurance; in Finland nurse appointments at public health centres are free; in Lithuania the mental health centre is free to registered insured people; in Norway the municipal service is free. It is the cheapest funded mental-health appointment in each of those systems.

Can a mental-health nurse prescribe?

No. The nurse works alongside the doctor who prescribes, monitors how a medicine is settling and reports back. Starting, changing or stopping a medicine remains the prescriber’s decision.

Who does what

What a psychologist does

Five or six years of university training, assessment as the core skill, no prescription pad — and a title that is protected in some of these countries and not in others.

What a psychotherapist is

A long post-qualification training in a named method, sat on top of another profession — and the one title in this field that is sometimes anybody’s to use.

What a psychiatrist does

A medical doctor with a specialist qualification: diagnosis, medication, physical causes, sick leave and hospital care — usually in shorter appointments than people expect.

Counselling, peer support and everything that is not therapy

Helplines, counsellors, peer groups, chaplains, self-help programmes and employer schemes: what each is genuinely good at, and where each stops.
