How long therapy takes to work

When to expect the first change, what a fair review point looks like, and what to do when nothing is moving.

The shape of improvement

Change in therapy is usually not linear and usually not dramatic. The common pattern is a small early lift from being taken seriously and having a plan, then a plateau, then slower structural change over weeks. Early sessions can also make things feel temporarily worse, particularly where the work involves approaching what has been avoided — which is the treatment functioning rather than failing, and one of the main reasons people drop out at exactly the point the treatment starts to do something. A therapist who tells you this in advance is saving you from that exit.

A fair review point

For structured therapies, six to eight sessions is the conventional point at which something should be visible — not resolution, but movement in the measures, in function or in the frequency of the thing you came about. Agree this at the first session: what we expect to see, by when, and what we will do if we do not see it. That single agreement converts a vague sense of drift into a scheduled decision, and it is the most useful piece of housekeeping in the whole course.

What "it is not working" usually means

Rarely that therapy cannot help. More often one of five specific things. The problem is not what the therapy is treating — an undiagnosed physical illness, a sleep disorder, alcohol, or a diagnosis that has been missed. The method is wrong for the problem, which is fixable by changing the method rather than the person. The dose is wrong: fortnightly sessions for a problem that needs weekly, or eight sessions for something structural. Something outside is overwhelming the work — an unsafe home, a job that is causing the illness, poverty, an ongoing crisis. Or the fit is poor, which is a real and legitimate reason, and not a criticism of either party.

Medication, timelines and honesty

Where medication is part of the plan the timelines are different and worth knowing so that a normal course is not mistaken for failure. Most antidepressants take weeks rather than days to show an effect, and the first fortnight can bring side effects before any benefit, which is the period in which most people stop. Judgement earlier than the prescriber’s stated review is unreliable. Everything about starting, changing or stopping belongs to the prescriber, and if you want to stop, the conversation is about how — abrupt discontinuation of psychiatric medicines can be genuinely harmful.

The end, and after

Good therapy ends deliberately: a review, a summary of what changed, an explicit plan for relapse, and sometimes spaced follow-up sessions rather than a hard stop. Ask for that ending rather than letting the course dissolve. Ask specifically what your early warning signs are and what you should do when you notice them, because a written relapse plan made while you are well is far better than one improvised while you are not. And recognise that a second course later is a normal event in the life of a recurring condition, not evidence that the first one failed.

Preguntas frecuentes

How long before therapy starts to help?

For structured therapies, six to eight sessions is a conventional point at which some movement should be visible in the measures or in function. Early sessions sometimes feel worse, especially when the work involves approaching what has been avoided, and that is the treatment working rather than failing.

How do I know if it is working?

Agree at the first session what you expect to see and by when, and use the same short questionnaires at the start and at the review. Memory for how bad things were is unreliable in both directions, which is exactly why the repeated measure exists.

Therapy is not helping. What should I change?

Check five things before concluding therapy cannot help: whether a physical cause, sleep disorder or alcohol is driving it; whether the method fits the problem; whether the frequency and length are enough; whether something outside is overwhelming the work; and whether the fit is right.

When should I stop?

At a planned ending with a review, a summary and a written relapse plan — not by drifting away. Ask what your early warning signs are and what to do about them. Needing a further course later is normal in a recurring condition.
