Counselling, peer support and everything that is not therapy
Helplines, counsellors, peer groups, chaplains, self-help programmes and employer schemes: what each is genuinely good at, and where each stops.
A layer people skip past
Between doing nothing and starting psychotherapy there is a wide band of real, useful, mostly free support, and a large share of the people searching for a therapist would be better served — or better served first — by something in it. This is not a consolation prize. Structured self-help with guidance has evidence behind it for mild to moderate anxiety and depression; peer support does things a professional relationship cannot; and a counsellor working on one concrete present problem for six sessions is frequently the correct dose rather than a smaller one.
Counselling versus psychotherapy
The words overlap and no border in the region draws the line the same way, but the working distinction is useful. Counselling tends to be shorter, focused on a present difficulty — a bereavement, a divorce, a job loss, a decision — and works with a person who is essentially well and overwhelmed. Psychotherapy tends to be longer, addresses patterns rather than episodes, and is designed for a diagnosable condition. Trouble arises in one direction only: counselling offered where a disorder needs treating wastes months. The reverse — a short, focused course for a situational problem — is usually good practice, not underselling.
Helplines are treatment-adjacent, not treatment
A crisis line answered by a trained volunteer at two in the morning is doing something no clinic does, and doing it well: it holds the hours that clinics do not cover. What it does not do is diagnose, treat or follow up. Use it for the hours, for the anonymity, and for talking to someone when talking is the thing that is needed; do not use it as a substitute for an assessment you have been putting off. The numbers for each country are on the crisis page.
Peer support and lived experience
Peer support — user organisations, condition-specific groups, bereavement groups, groups for family members — offers something professional care structurally cannot: someone who has been through it, and the demonstration that the state you are in has an exit. It is particularly strong for long conditions, for carers, and for the isolation that comes with both. It is not supervised clinical treatment, quality varies with the group, and a group organised around a shared grievance can entrench rather than help. National mental-health organisations in each country run or list groups, and their sites are the sensible starting point.
Guided digital programmes
Structured internet-delivered cognitive-behavioural programmes, particularly with a therapist attached, have a real evidence base for mild to moderate anxiety and depression. Finland has made this public infrastructure: the Mielenterveystalo portal carries open self-help programmes and doctor-referred guided net therapies, free of charge. Elsewhere the offering is mixed public and commercial. The distinction worth holding is between a programme with a named clinician attached to your progress and an app that is a mood diary with a subscription.
Chaplaincy, employers and municipalities
Three more doors that people forget. Pastoral and chaplaincy services offer conversation without a diagnosis, to anyone, usually free, and in several of these countries they run the emotional-support telephone lines. Employers often buy an employee assistance scheme with a small number of confidential counselling sessions, which the employee has typically never been told about — the page on getting help through work covers how to find out. And municipal social services run family, crisis and debt counselling whose availability varies by municipality but which is free where it exists, and which is frequently the right service for a problem that is a life crisis rather than an illness.

Questions fréquentes
Is counselling as good as therapy?
For a present, situational difficulty in someone who is otherwise well, a short focused course of counselling is often exactly the right treatment rather than a lesser one. For a diagnosable disorder, it is not a substitute for psychotherapy or medical treatment.
Do self-help programmes actually work?
Structured internet-delivered cognitive-behavioural programmes have real evidence for mild to moderate anxiety and depression, and the effect is stronger when a clinician is attached to your progress. An unguided mood-tracking app is a different product with a much weaker claim.
What can a helpline do for me?
Answer at the hours nothing else is open, without an appointment, without your name, and give you a trained person to talk to. It cannot assess, diagnose, treat or follow up, so it works best alongside a plan rather than instead of one.
Does my employer offer counselling?
Many do and never mention it. Ask occupational health or human resources whether there is an employee assistance scheme; in Finland occupational health itself often includes short psychological support and is free to the employee.
Who does what
What a psychologist does
Five or six years of university training, assessment as the core skill, no prescription pad — and a title that is protected in some of these countries and not in others.
What a psychotherapist is
A long post-qualification training in a named method, sat on top of another profession — and the one title in this field that is sometimes anybody’s to use.
What a psychiatrist does
A medical doctor with a specialist qualification: diagnosis, medication, physical causes, sick leave and hospital care — usually in shorter appointments than people expect.
The mental-health nurse
The fastest funded appointment in several of these countries, and the one most people have never heard of.
