Confidentiality, records and what gets written down
Who can see your notes, the four situations where confidentiality has limits, and what a mental-health record does and does not affect.
The rule and its four exceptions
Everything you say in a clinical mental-health setting is confidential, and the duty is a legal one that outlives the treatment. The exceptions are narrow and broadly the same across all seven countries: a serious and immediate risk to your own life; a serious risk to someone else; a concern that a child is being harmed; and a court order or a specific statutory duty. Notice what is not on that list — being unwell, having suicidal thoughts in general, using drugs, or saying something you are ashamed of. A good clinician sets out these limits at the first session, and if they do not, asking is entirely reasonable.
Who can actually read the notes
This differs by country and by setting, and the difference matters. Inside public health care, notes are part of a health record that other treating clinicians can generally access when they are involved in your care; Estonia, Denmark and Finland run national patient portals through which you can see much of your own record and, in some cases, control parts of the sharing. A private therapist outside the public system usually keeps their own records, which are not in the national system at all. Your employer cannot read any of it. Your family cannot read any of it without your consent, unless you are a child or lack capacity to decide.
Sick leave says less than you fear
A sick-leave certificate is written by a doctor, goes to the employer or the benefits authority, and confirms incapacity for work and its expected duration. In this region the employer is generally not entitled to the diagnosis — that goes to the health or insurance authority rather than to the manager. This is the single most common worry that stops people asking for help, and it is largely misplaced: the certificate proves you are unfit for work, not what is wrong with you. What you tell your own manager beyond that is your choice, and the page on getting help through work covers how to decide.
Driving licences, firearms and security clearance
These are the real, specific consequences, and vagueness about them is unhelpful in both directions. Medical fitness to drive is regulated in every country here, and certain conditions and certain medicines can trigger an assessment or a limitation — the duty is usually on the doctor and sometimes on the driver to report. Firearms licensing and some security clearances involve health checks. None of this follows automatically from having had therapy or from taking an antidepressant; it follows from specific conditions, specific medicines and specific licensing rules. If it applies to you, ask the treating doctor the question directly rather than guessing, because the answer is jurisdictional and knowable.
Practical control over your own record
Three things are worth knowing. You generally have a right of access to your own health record, through a national portal or on request, and reading it is instructive. You can ask what is being written and how it will be phrased — a reasonable request, and clinicians can often record something accurately in more than one way. And if the record contains a factual error, there is a correction procedure in every country here; an error in a health record is worth fixing at the time rather than after it has been copied into three other documents.

Perguntas frequentes
Can my therapist tell anyone what I said?
Only in narrow circumstances: a serious and immediate risk to your life, a serious risk to someone else, a concern that a child is being harmed, or a court order or specific legal duty. Being unwell, having suicidal thoughts in general or using drugs are not in themselves exceptions.
Will my employer find out?
No. A sick-leave certificate confirms incapacity for work and its duration; in this region the employer is generally not entitled to the diagnosis, which goes to the health or insurance authority. What you choose to tell a manager beyond that is up to you.
Does a mental-health record affect my driving licence?
Not by itself. Medical fitness to drive is regulated and specific conditions or medicines can trigger an assessment or restriction, but having had therapy does not. If it might apply to you, ask the treating doctor directly, because the rules are jurisdictional and knowable.
Can I see what my therapist wrote?
Generally yes — there is a right of access to your own health record in every country here, often through a national patient portal. You can also ask how something will be phrased, and there is a correction procedure if the record contains a factual error.
