Changing therapist, and ending well
How to tell a bad fit from a hard patch, how to raise it, and how to switch inside a funded system without losing your place.
A hard patch and a bad fit look similar from inside
Therapy that is working often feels worse for a while: approaching what has been avoided is unpleasant by design, and the middle of a course is where most people want to leave. Therapy that is not working can feel identical from the inside. The distinguishing questions are about direction rather than comfort. Is the difficulty connected to the material, or to the person? Do you leave with something to think about, or only with relief that it is over? Has anything measurable moved since session one? And can you imagine saying this to the therapist? If the answer to the last one is no, that is itself the finding.
Say it in the room first
Raising dissatisfaction directly is not rudeness and it is not a waste of a session; it is often the most productive thing that happens in a course. In several traditions the repair of a rupture in the working relationship is regarded as therapeutic work in its own right, and a competent therapist will treat it as material rather than as a complaint. Say it plainly: this does not feel like it is helping, here is what I expected, here is what is happening instead. What you learn from the response is worth having either way — a therapist who can take it is probably worth staying with, and one who becomes defensive has answered your question.
Reasons to leave without a conversation
Some things are not a rupture to be repaired. Feeling judged, mocked or dismissed. Any social, financial or romantic contact outside the therapy. Being told to stop a prescribed medicine. Pressure to commit to long, prepaid blocks. Breaches of confidentiality. Advice that your family or your doctor must not know about the treatment. Any of these is a reason to stop immediately, and several of them are also reasons to report — the complaints page lists where, by country.
Switching inside a funded system
The practical rules differ and it is worth knowing yours before you act. In a public service you are usually asking to change clinician within the same service, which is a normal request made to the service rather than to the therapist. Under Denmark’s subsidy scheme and Norway’s agreement system the entitlement attaches to the referral rather than to the individual, so moving practice is usually possible without starting the referral again — but confirm it before you cancel anything. Under Finland’s Kela scheme the decision names conditions rather than a person, and a change of therapist has its own procedure. In every case, ask first and cancel second, because the sequence protects the entitlement.
Ending on purpose
Whether you are finishing or moving on, a deliberate ending is worth one session. Review what changed and what did not. Ask for a written summary you can take with you, which makes the next assessment enormously better and prevents you from having to tell the whole story from the beginning again. Ask what your early warning signs are and what the plan is if they appear. And if you are switching rather than stopping, ask whether the notes can be shared with the next clinician, with your consent — continuity is worth more than a clean break.

Perguntas frequentes
How do I know whether to change therapist?
Ask whether the difficulty is with the material or with the person, whether anything measurable has moved since the first session, and whether you could imagine raising it in the room. If you could not raise it, that answer is itself the finding.
Should I tell them?
Yes, in almost every case. Raising it directly is often the most productive session in a course, and repairing a rupture is treated as therapeutic work in several traditions. The exception is behaviour that is a boundary breach rather than a difference of approach.
Will I lose my place if I switch?
Usually not, if you ask before you cancel. In several systems the entitlement attaches to the referral or the decision rather than to the individual therapist, but the procedures differ, so confirm with the service or the scheme first and cancel afterwards.
What should I ask for when a course ends?
A review of what changed, a written summary you can take to the next clinician, your early warning signs, a plan for what to do if they appear, and — if you are switching rather than stopping — consent-based sharing of the notes with whoever comes next.
