Getting help through work

Occupational health, employee assistance schemes and sick leave: the fastest route in the region for an employed person, and the one most employees never open.

This is a map of how the systems work, not a diagnosis and not a treatment plan. It names no practice and no private price, because neither can be kept accurate here. It cannot tell you what is wrong or which treatment you need — that is a clinician's job, and it starts with an appointment. Never stop or change a prescribed medicine on the strength of a web page. If someone's life is in danger right now, call the emergency number: 112 anywhere in the EU and EEA.

Occupational health is a real health service

In Finland especially, occupational health is not a formality: employers are required to arrange it, many buy more than the statutory minimum, and for an employed person it is frequently the fastest route to a psychological appointment in the country — free to the employee, with no referral needed to enter, and with the ability to refer onwards. The other countries here run narrower versions, more focused on workplace risk and fitness for work, but all of them include the occupational health professional as someone you may consult about how work and health are interacting. If you are employed and you do not know what your occupational health provider offers, that is the single most valuable phone call available to you.

What the employer does and does not see

The confidentiality question is what stops most people, and the answer is more protective than expected. Occupational health professionals are health professionals bound by clinical confidentiality; the employer receives conclusions about fitness for work and any adjustments needed, not the clinical content. A sick-leave certificate confirms incapacity and duration, and in this region the diagnosis generally goes to the health or insurance authority rather than to the manager. Employee assistance schemes bought by an employer are normally delivered by an external provider precisely so that the employer sees usage statistics rather than names.

The scheme you were never told about

A large number of employers pay for an employee assistance programme offering a small number of confidential counselling sessions, often extending to family members, and mention it once at induction and never again. It costs the employee nothing, needs no referral, and is usually available within days. The way to find out is to ask human resources or occupational health directly whether an employee assistance or counselling benefit exists, or to look at the benefits documentation you were given when you started. It is not a substitute for treatment of a disorder, but for a specific crisis it is fast and free.

Sick leave that helps rather than hides

Time off is a treatment decision rather than a moral one, and it works when it is used for something. Open-ended leave with no plan and no contact tends to make return harder, while a period with a stated purpose — starting treatment, stabilising sleep, a course of therapy — and a planned review works better. Graded or partial return to work exists in several of these countries and is worth asking about specifically, because returning at half strength is often the difference between a return that holds and one that fails in a fortnight. The doctor writing the certificate is the person to have this conversation with.

When work is the cause

Sometimes the honest formulation is that the job is producing the illness — through workload, bullying, an impossible role, or shift patterns that make sleep impossible. Therapy will not fix a workplace, and framing an organisational problem as an individual’s coping deficit is a well-recognised failure mode. Occupational health is the correct address for this because it is the one service with a mandate over both sides, and every country here has workplace health-and-safety obligations that cover psychosocial risk. Where the pattern is shift work and disturbed sleep, the mechanism is worth understanding on its own terms — the chronotype pages cover the body-clock side, and the workplace surface covers what employers can change.

Frequently asked questions

Will my employer know if I use occupational health?

They receive conclusions about fitness for work and any adjustments needed, not the clinical content. Occupational health professionals are bound by clinical confidentiality in the same way as any other health professional.

Is occupational health really the fastest route?

For an employed person in Finland it usually is, and it is free to the employee. In the other countries the service is narrower but still includes consultation about how work and health interact, and it can refer onwards.

What is an employee assistance programme?

An employer-funded benefit giving employees, and often their families, a small number of confidential counselling sessions through an external provider. It is free, fast and needs no referral. Ask human resources or occupational health whether one exists — most employers mention it once and never again.

What if my job is what is making me ill?

Take it to occupational health, which is the one service with a mandate over both the health and the workplace. Therapy cannot fix an organisational problem, and treating workload, bullying or impossible shift patterns as an individual coping deficit is a recognised failure mode.
